- Applicants who hold a Puerto Rico license and have no other Active Unrestricted Physician Assistant license, do not qualify for the Universal Recognition License.
Basic Requirements for Universal Recognition License

• You *MUST* have an Active license in another State for at least a *period of one year*
• Provide proof of residency in Arizona or Military Form 2058
